Athletic Forms
Largo, Md. – Student Athletic forms are available online for new and returning Owls for the 2016-17 season. All student athletes wishing to participate in practices or tryouts must have the following forms completed and returned to the Athletic Trainer located in Novak Field House Room 123 at least three days prior to participation. Forms can also be dropped off in Novak 111.
- Physical Form (must be signed and stamped by doctor)
- Personal Data Form
- Student-Athlete Concussion Reporting Agreement
- Potential Student Athlete Data Form
- Copy of current class schedule or paid bill (printed from Owl Link)
Athletes must submit a Web Bio Form at the beginning of their season for the athletic department to update the website and rosters. At the end of their seasons athletes are asked to submit an evaluation of their season, coaches, athletic experience, etc. Those forms are below.
