The completion of this form is one of the first steps to participating on an intercollegiate athletic team at Prince George's Community College. This form must be completed in its entirety and as ACCURATELY as possible. Intentionally falsifying or omitting information can make you ineligible for ALL future college competitions in compliance with the National Junior College Athletic Association (NJCAA) eligibility rules.
