Athletic Forms

*Any student-athlete wishing to participate in intercollegiate athletics during the 2022-23 academic year must provide either proof of full vaccination status OR show proof of vaccine exemption from the college. If exempt, student athletes must still provide a negative COVID test weekly prior to any participation. Negative tests must be submitted every week for the entire duration of the season to include preseason, regular season and post-season play. Below are frequently requested forms for student athletes. 

All student athletes wishing to participate in practices or tryouts must have the following forms completed and returned to the Athletic Trainer located in Novak Field House Room 123 at least three days prior to participation.

  • Physical Form (must be signed and stamped by doctor. Lab results alone are NOT the same as a sports physical)
  • Potential Student Athlete Data Form
  • Copy of current class schedule or paid bill. To be eligible to participate, a student must be enrolled as a full-time student with 12 credits (printed from Owl Link)

Athletes must submit a Web Bio Form at the beginning of their season for the athletic department to update the website and rosters. At the end of their seasons athletes are asked to submit a evaluation of their season, coaches, athletic experience, etc. Those forms are below.

Student Athlete Concerns Form

The College is committed to an athletic environment that values all individuals and groups. The athletic programs does not tolerate discrimination on the basis of race, religion, color, sex, sexual orientation, national origin, ancestry, age, gender identity or expression, mental or physical disability, or veteran status. In addition, the department seeks to provide fair and equitable treatment in all areas related to a student-athlete’s well-being.

The College has created an online process through which students can share concerns related to, but not limited to, Mental and Physical well-being, Health and Safety Protocols, Team Culture including abusive behavior, harassment or hazing by a coach, a college staff member or other student-athlete.  Concerns submitted through this process will be reviewed by a senior level administration, who is not directly involved with the management of athletics programs. 

Please note that this process is not intended to address such concerns as playing time and position on the team. There are separate appeal processes for violations of the substance abuse policy, student-athlete conduct policy and issues related to NJCAA rules.  These issues should be discussed with the Athletics Director.
